Much as the NHS is a shit-show at the moment, this is one of the rare instances where they've got it right. NICE (National Institute for Clinical Excellence) did the literature review and recommended prescribing HRT at GP level (no need for a referral to a gynaecologist) based on symptoms alone (no need for blood tests which don't work in perimenopause anyway because the whole point of peri is your hormone levels fluctuate wildly, so a one-off blood test tells you exactly fuck all).
Of course there are individual GPs still stuck 20 years ago who won't prescribe, but for the most part in my experience they're pretty good (I've only ever had a problem with one locum).
I'm surprised premarin is still being used in Canada (that's one of the conjugate equine oestrogens, isn't it?) I don't think it's used anywhere in western Europe any more.
